The first two examples are Category-3 2-jack units installed before 1993. "Category-3" means the wiring and jacks are rated for data speeds of 10 megabit/sec or less.

The next 2 examples are Category-5 3-jack units installed after 1993. "Category-5" means the wiring and jacks are rated for data speeds of 100 megabit/sec or less. They also include a third "network" jack in addition to the "data" and "voice" jacks. The "network" jack is not usable in some buildings because its cable doesn't go to the place where the network equipment is.

This is a Cat5 4-jack wall plate cluster from the ITRP upgrade.
"A" is the telco closet room number. The building name is not given on the label, nor is the room number where this cluster is located. Since there are many instances of "T4001" on campus (about as many as there are 4-story buildings), you need to tell us the building and room where this cluster is for us to know which one you mean. This one happens to be located in ARC 4016.
"B" is the number of the upper-right jack. This jack's full number is ARC T4001 AD23. Jacks are numbered from AA01 through AA48, AB01 through AB48, etc. The jacks are "universal", meaning that any jack can support any service. Typically voice services start at the lowest-numbered jack in the cluster (AD23 in this case) and go up, net services start at the highest-numbered jack in the cluster (AD26 in this case) and work down. AA48 is a lower number than AB01.
"C" is a cover over an empty jack slot. A jack can be installed there, but there is no jack behind the cover, and there is no unterminated cable behind the cover.
